Embarking on a weight loss journey is an important step for overall health and wellness, and losing a significant amount of weight can change how your body looks and feels.
“Weight loss is a big accomplishment,” WVU Medicine Plastic Surgeon Jack Gelman, MD, said. “But sometimes after weight loss, issues like excess hanging skin, separated muscles, and remaining fat can hinder people from feeling comfortable in their bodies.”

Panniculectomy
A panniculectomy is a skin removal surgery targeting the midsection, specifically to remove a pannus or an apron belly – stretched out skin and fatty tissue from the lower abdomen hanging over the pubic area.
“Panniculectomies are designed to remove the excess hanging skin that can come after major weight loss,” Dr. Gelman said. “These procedures aren’t just cosmetic.”
Often an insurance-based procedure, panniculectomies remove this excess skin to relieve physical discomfort, address chronic rashes that may form in skin folds, and make hygiene practices easier.
“We see a lot of patients who have undergone bariatric surgery be prime candidates for a panniculectomy,” Dr. Gelman said. “It can really be lifechanging and help make weight loss transformations more comfortable for people.”
Tummy Tuck
Tummy tucks, or abdominoplasties, aim to create a firmer midsection through removing fat and tightening skin and muscles.
“A tummy tuck goes a step beyond a panniculectomy because we’re not just removing skin, but we’re also removing soft tissue, tightening abdominal muscles, repositioning the belly button, and creating a slimmer and smoother waistline,” Dr. Gelman said.
Tummy tucks can be done on their own, or as part of a “mommy makeover,” which is any combination of procedures involving the abdomen and breasts – like lifts, reductions, and augmentations. In addition, some patients choose to add a tummy tuck onto an insurance-based panniculectomy for further contouring.
“A tummy tuck can make a real difference after pregnancy and childbirth,” Dr. Gelman said. “Abdominal muscles separate and weaken during pregnancy, and during this surgery we can firm and tighten those muscles in a targeted way most exercises just can’t do.”
Brachioplasty
A brachioplasty, sometimes called an arm lift, removes excess fat and sagging skin from the upper arms.
“The upper arms are a common problem area after weight loss,” Dr. Gelman said. “A decrease in skin elasticity is a normal part of aging, which can lead to a drooping or sagging flap of skin.”
While exercises like weightlifting and resistance training can strengthen and tone upper arm muscles, these workouts alone can’t tighten loose skin.
“In a brachioplasty, we can remove excess skin, smooth remaining skin, and tighten tissue for a more sculpted and toned appearance,” Dr. Gelman explained.
