Discharge
As your health improves, we will work with you and your family to prepare for a safe homecoming.
Care Management will arrange and coordinate post-acute needs that may include inpatient rehab, skilled nursing, home care, equipment, and any specialized medications. Nursing will provide you with discharge instructions, discuss any new medications or medication changes, and inform you of follow-up appointments. The WVU Medicine Transition of Care Clinic provides patients with a bridge from J.W. Ruby Memorial Hospital to home when additional assistance may be needed.
The WVU Medicine Discharge Pharmacy offers in-house delivery to patients and mail delivery to patients’ homes. WVU Medicine also offers prescription services through its Medical Center Pharmacy and Mountaineer Pharmacy.

Support Services
WVU Medicine offers a variety of support services for patients and family members who are coping with a medical condition, including support groups, music therapy, and spiritual care.
Care Management can help with any personal, financial, or family problems that may arise. Staff members are available for counseling and can help with understanding and obtaining community resources. Call 304-598-4183 or ask your nurse for assistance.
