Discharge
Your Care Team will work with you to develop a transition plan to fit your needs. We believe in promoting a successful recovery through physical, emotional, and mental support.
You will receive written instructions concerning medications, activities, appointments, etc., prior to discharge. Should you need follow up outpatient testing, you will receive information on how to schedule these procedures through the Hospital’s Community Wide Scheduling Department. If you have questions, please don’t hesitate to ask your Care Team.
